- Billing information (if paying by Credit Card, please make sure this information matches your credit card account) including name, address, phone and email address.
- Please indicate which EFCA church you attend or are employed at (if applicable), including City and State.
- Add an attendee (and spouse if applicable). All attendees (even if only 1) need to be added here. When adding attendees, indicate whether you are attending one day (Monday or Tuesday) or both days, or just the Dinner on Monday Evening. If you are registering your spouse as well, please indicate the same for them. Then click “Add Attendee (and spouse)” to complete the entry of the individual/couple. You will return to the main form. If needed, you can add more attendees.
- Indicate payment by Credit Card/Paypal or Check
- If paying by Credit Card, you may optionally add an amount to cover the PayPal transaction fee.
- Click on the Captcha box, and if necessary solve the puzzle provided (this is to prevent “robotic” spam submissions).
- Click “Submit”. If you are paying by Credit Card/Paypal, you will be presented with the Paypal Payment screen, where you can complete the payment portion of your registration.
- You will receive an email confirming your registration. If paying by Check, please mail a check to the District Office (823 Wheeler Street, Suite 10, Ames, IA 50010) within 7 days for the indicated amount.
